Cardiology Patient Navigator

Location: Remote · Global; Conditions: Manila, Metro Manila, Philippines. Must work consistently during US business hours in Central Time (CT).

Salary: $5–7 per hour.

What you will do

• Manage patient scheduling, reminders, follow-ups, and care coordination workflows in EMR/EHR systems.
• Conduct telephonic and remote outreach to reinforce care instructions and coordinate next steps.
• Support cardiology care pathways, including CHF management protocols and remote patient monitoring.
• Track testing schedules, appointments, and follow-up activities across a caseload of 150+ patients.
• Maintain accurate clinical documentation and support HIPAA-compliant patient records, billing documentation, and insurance coordination.
• Escalate clinical concerns to the appropriate provider without diagnosing or prescribing.

Requirements

• Clinical healthcare background as a CNA, MA, RN, or equivalent credential.
• At least 3 years of experience in healthcare, telehealth, or medical office settings.
• Experience with care coordination or care management, preferably in telephonic or remote environments.
• B2+ English proficiency, assessed during screening.
• Experience using EMR/EHR systems and strong patient-centered written and verbal communication skills.
• Availability for US business hours in Central Time (CT).

Nice to have

• Experience in cardiology, med-surg, ICU, cardiac step-down, CCU, or home health.
• Outpatient cardiology, home health, or telemedicine experience.
• Exposure to remote patient monitoring, CCM, utilization review, medical billing, or prior authorization.
• Previous experience in contractor or startup/evolving environments.
