Cardiology Patient Navigator (Cardiology)

Location: Remote and global; conditions list Bogotá, Colombia. Must work consistently during US business hours in Central Time.

Salary: USD 5–7 per hour

Healthcare organization supporting a US-based cardiology care team.

What you will do

• Manage patient scheduling, reminders, follow-ups, and care coordination workflows.
• Conduct telephonic and remote patient outreach to reinforce care instructions and coordinate next steps.
• Support cardiology care pathways, including CHF management protocols.
• Track testing schedules, appointments, and remote patient monitoring workflows across a caseload of 150+ patients.
• Maintain accurate clinical documentation and support billing and insurance coordination.
• Escalate clinical concerns to the appropriate provider without diagnosing or prescribing.

Requirements

• Clinical healthcare background as a CNA, MA, RN, or equivalent credential.
• At least 3 years of experience in healthcare, telehealth, or medical office settings.
• Experience with care coordination or care management, preferably in telephonic or remote settings.
• Experience using EMR/EHR systems and strong patient-centered communication skills.
• B2+ English proficiency is required and assessed during screening.
• Availability for US business hours in Central Time is required.

Nice to have

• Experience in cardiology, med-surg, ICU, cardiac step-down, CCU, home health, or outpatient cardiology.
• Experience with telemedicine, remote patient monitoring, CCM, or utilization review.
• Medical billing or prior authorization knowledge.
• Prior contractor experience or experience in startup and evolving environments.
