Cardiology Patient Navigator (Coordinator)

Location: Remote from Lima, Peru; U.S. business hours in Central Time required.

Salary: USD 5–6 per hour

What you will do

• Manage patient scheduling, reminders, follow-up workflows, and care coordination in EMR/EHR systems.
• Conduct telephonic and remote outreach to reinforce care instructions and coordinate next steps.
• Support cardiology care pathways, including CHF management protocols.
• Track testing schedules, appointments, and remote patient monitoring workflows for a caseload of 150+ patients.
• Escalate clinical concerns to the appropriate provider within scope, without diagnosing or prescribing.
• Maintain accurate clinical documentation and support billing and insurance coordination.

Requirements

• Clinical healthcare background such as CNA, MA, RN, or an equivalent credential.
• At least 3 years of experience in healthcare, telehealth, or medical office settings.
• Experience in care coordination or care management, preferably in telephonic or remote settings.
• B2+ English and B2+ Spanish proficiency are required.
• Experience using EMR/EHR systems and strong patient-centered communication skills.
• Ability to work U.S. business hours in Central Time.

Nice to have

• Experience in cardiology, med-surg, ICU, cardiac step-down, CCU, or home health.
• Outpatient cardiology, home health, or telemedicine experience.
• Remote patient monitoring experience.
• Medical billing or prior authorization knowledge.
• Contractor experience or experience working in startup and evolving environments.
