Cardiology Patient Navigator (Coordinator)

Location: Remote from Guatemala City, Guatemala; U.S. business hours (CST)

Salary: USD 5–6 per hour

hirify.global supports a U.S.-based cardiology team with patient care coordination and clinical operations.

What you will do

• Manage patient scheduling, reminders, follow-ups, and care coordination workflows in EMR/EHR systems.
• Conduct telephonic and remote outreach to reinforce care instructions and coordinate next steps.
• Support cardiology care pathways, including CHF management protocols and remote patient monitoring.
• Track testing schedules and appointments across a caseload of 150+ patients.
• Escalate clinical concerns to the appropriate provider without diagnosing or prescribing.
• Maintain clinical documentation, support billing and insurance coordination, and follow HIPAA requirements.

Requirements

• Clinical healthcare background such as CNA, MA, RN, or an equivalent credential.
• At least 3 years of experience in healthcare, telehealth, or medical office settings.
• Experience with care coordination or care management, preferably in telephonic or remote settings.
• B2+ English and B2+ Spanish proficiency are required; bilingual communication is assessed during screening.
• Experience using EMR/EHR systems and strong patient-centered written and verbal communication.
• Ability to work U.S. business hours in Central Time.

Nice to have

• Experience in cardiology, med-surg, ICU, cardiac step-down, CCU, home health, outpatient cardiology, or telemedicine.
• Experience with remote patient monitoring, medical billing, or prior authorizations.
• Previous contractor experience or experience in startup and evolving environments.
