Cardiology Patient Navigator (Cardiology)

Location: Remote · Global; Caracas, Capital District, Venezuela. Required availability during US business hours, Central Time (CT).

Salary: USD 5–7 per hour

Healthcare care coordination support for a US-based cardiology team.

What you will do

• Manage patient scheduling, reminders, follow-up workflows, and care coordination in EMR/EHR systems.
• Conduct telephonic and remote patient outreach to reinforce care instructions and coordinate next steps.
• Support cardiology care pathways, including CHF management protocols.
• Track testing schedules, follow-up appointments, and remote patient monitoring workflows for a caseload of 150+ patients.
• Maintain accurate clinical documentation, support billing and insurance coordination, and follow HIPAA requirements.
• Escalate clinical concerns to the appropriate provider within scope without diagnosing or prescribing.

Requirements

• Clinical healthcare background as a CNA, MA, RN, or equivalent credential.
• At least 3 years of experience in healthcare, telehealth, or medical office settings.
• Experience with care coordination or care management, preferably in telephonic or remote settings.
• B2+ English proficiency with written and verbal communication assessed during screening.
• Experience using EMR/EHR systems and strong patient-centered communication skills.
• Ability to work consistently during US business hours in Central Time.

Nice to have

• Experience in cardiology, med-surg, ICU, cardiac step-down, CCU, home health, outpatient cardiology, or telemedicine.
• Exposure to remote patient monitoring, CCM, utilization review, medical billing, or prior authorization.
• Previous experience in contractor or startup/evolving environments.
