Cardiology Patient Navigator (Coordinator)

Location: Remote from Caracas, Capital District, Venezuela; must work U.S. business hours (CST)

Salary: USD 5–6 per hour

What you will do

• Manage patient scheduling, reminders, follow-ups, and care workflows in EMR/EHR systems.
• Conduct telephonic and remote outreach to reinforce care instructions and coordinate next steps.
• Support cardiology care pathways, including CHF management protocols and remote patient monitoring.
• Coordinate testing schedules, appointments, and continuity of care for a caseload of 150+ patients.
• Escalate clinical concerns to the appropriate provider without diagnosing or prescribing.
• Maintain compliant clinical documentation and support billing and insurance coordination.

Requirements

• Clinical healthcare background such as CNA, MA, RN, or an equivalent credential.
• At least 3 years of experience in healthcare, telehealth, or medical office settings.
• Care coordination or care management experience, preferably in telephonic or remote settings.
• B2+ English and B2+ Spanish proficiency are required.
• Experience with EMR/EHR systems and strong patient-centered communication skills.
• Availability during U.S. business hours (CT).

Nice to have

• Experience in cardiology, med-surg, ICU, cardiac step-down, CCU, home health, outpatient cardiology, or telemedicine.
• Remote patient monitoring experience.
• Medical billing or prior authorization knowledge.
• Previous contractor experience or experience in startup and evolving environments.
