Location: Remote, with candidates based on the U.S. West Coast preferred and approximately 20–25% annual regional travel to partner sites across Northern California and the Seattle, Washington area. The role also states that on-site presence may be required 3–4 days per week in Shreveport, Louisiana, subject to practice needs.
Salary: $95,000–$100,000 per year base, plus bonuses and equity.

What you will do
- Build strategic relationships with physicians, providers, and healthcare executives.
- Manage financial, quality, clinical, population health, care management, HEDIS, STARs, and operational performance across assigned accounts.
- Use analytics tools to identify performance gaps, communicate results, and develop project plans.
- Coach practice staff and providers on value-based care processes, technologies, and contract objectives.
- Prepare executive-facing presentations and memos covering practice performance, financial outcomes, projections, and contract terms.
- Surface root causes affecting performance and coordinate with internal and external stakeholders.
Requirements
- 6–8 years of relevant experience in practice management, project management, account management, analytical problem-solving, and healthcare strategy.
- At least 5 years of executive-level written and verbal communication experience.
- Demonstrated knowledge of value-based care contracts across patient types.
- Ability to travel for multi-day, in-person client visits in Northern California and the Seattle, Washington area.
- Ability to work on site 3–4 days per week in Shreveport, Louisiana when required by practice needs.
- Advanced skills with PowerPoint, Excel, data visualization, workflow analysis, and process mapping tools.
Nice to have
- Degree in nursing, social work, health technology, information technology, or a related field.
- Significant medical practice management experience may substitute for educational requirements.
- Strong organizational, project management, and team-building skills.
