Care Coordinator II

Location: Field-based role serving Izard, Sharp, and Fulton Counties, Arkansas. Candidates must reside in one of these counties; travel is approximately 30%.

Salary: $17.84–$28.02 per hour

What you will do

• Conduct member outreach by phone and through home visits to discuss care plans, services, resources, and next steps.
• Coordinate care activities with members, caregivers, healthcare providers, community organizations, and internal care teams.
• Respond to member and provider questions or concerns related to care and service plans.
• Support service assessments and screenings, identify member needs, and make referrals to care managers, disease managers, and community organizations.
• Document and maintain member records, care needs, referrals, and services in accordance with regulatory and contractual requirements.
• Provide education about available benefits and resources while supporting continuity of care.

Requirements

• High school diploma or GED and 1–2 years of related experience.
• For the Arkansas Total Care plan: a bachelor's degree in social science or a health-related field, or a high school diploma with at least one year of experience coordinating care for clients with developmental or intellectual disabilities or behavioral health needs.
• Must reside in Izard, Sharp, or Fulton County, Arkansas, or in an Arkansas border city.
• Valid driver's license required; the role includes approximately 30% travel and home visits.
• Child and adult maltreatment checks and drug screening are required before hire and on a recurring basis.
