Community Resource Coordinator II

Location: Field-based in Bladen, Brunswick, Columbus, Cumberland, Harnett, Hoke, Lee, Montgomery, Moore, New Hanover, Pender, Richmond, Robeson, Sampson, or Scotland County, North Carolina. Home visits and in-person community outreach are required. Schedule: Monday through Friday, 8:00 AM–5:00 PM, with evening and weekend work required.

Salary: $17.84–$28.02 per hour

What you will do

• Connect members with community and care resources in a cost-effective manner.
• Coordinate community outreach resources and promote available care and services.
• Conduct telephonic and in-person outreach, including visits to homes and community organizations.
• Perform non-clinical health, home-safety, transportation, employment, and community-resource assessments.
• Provide education, informal guidance, and member support while assisting nurses and social workers.
• Document community resources, correspondence, educational materials, referrals, and member support activities.

Requirements

• High school diploma or GED.
• 1–2 years of related experience.
• Residence in one of the specified North Carolina counties is required.
• Field-based and community outreach experience, including home visits.
• Computer proficiency, strong communication and customer service skills, and effective assessment abilities.
• Ability to work independently and as part of a team, including evening and weekend work.
