Physical Therapist - Innovative Mobile Rehab Ortho Neuro Part-time (Neuro/Ortho)

Location: Wheaton, Illinois, United States; mobile outpatient care is delivered directly in patients’ homes.

Salary: USD 41.50–56.03 per hour

What you will do

• Deliver outpatient physical therapy in patients’ homes and provide support through telehealth.
• Manage patient care from initial contact through discharge, including evaluations, re-evaluations, treatment planning, and outcome tracking.
• Select appropriate treatment modalities, equipment, and protocols, adapting care to individual patient needs.
• Document evaluations, daily notes, progress notes, and discharge summaries in the electronic medical record.
• Collaborate with PTAs, physicians, nurses, case managers, and other healthcare professionals.
• Participate in committees, process improvement, education, mentoring, and clinical student programs.

Requirements

• Bachelor’s Degree in Physical Therapy.
• Licensed Physical Therapist in the State of Illinois.
• BLS certification from the American Heart Association upon hire.
• Strong neurological and orthopedic physical therapy background.
• Adaptability across different care settings and willingness to adjust the schedule to meet patient-care needs.
• Proficiency with clinical documentation and electronic medical records.

Nice to have

• Master’s or Doctoral Degree in Physical Therapy.
