Behavioral Health Counselor-Community Support

Location: 631 S. First St., DeKalb, Illinois, United States. Full-time, daytime schedule, Monday–Friday, 40 hours per week. The role includes field work in clients' homes and communities, including transportation to medical and psychiatric appointments.

Salary: $27.30–$37.15 per hour

What you will do

• Screen and evaluate service requests, collect social histories, and assess client and family needs.
• Develop, implement, and evaluate individualized treatment and rehabilitation plans with clients and an interdisciplinary team.
• Provide supportive counseling, case management, advocacy, outreach, skills training, social rehabilitation, and community support.
• Intervene in and stabilize acute crisis situations while supporting clients with moderate to severe mental illness and co-occurring disorders.
• Connect clients with medical, behavioral health, government, and other human services programs.
• Support discharge planning, coordinate with agencies, and train providers and non-providers on relevant service specialties.

Requirements

• Bachelor's degree in Behavioral Health.
• Valid Illinois driver's license, current auto liability insurance, and an annual driving abstract required at hire and annually.
• Advanced verbal and written communication skills.
• Ability to provide community and in-home services and transport clients to medical or psychiatric appointments.
