PA or NP

Location: Lost Hills, California, USA, with weekly travel to the Delano clinic and mobile clinic locations

Salary: $150,000–$175,000 per year, plus eligibility for a discretionary bonus

What you will do

• Provide compassionate, high-quality patient care using evidence-based medical practices.
• Work in a multidisciplinary clinical environment to improve health outcomes for empaneled patients.
• Collaborate with physicians and health coaches on wellness goals, patient activation, and self-efficacy.
• Support onsite and community-based patient engagement, access, population management, referral tracking, and clinical quality evaluation.
• Provide care in clinic, community, and mobile-clinic settings, including outlying agricultural plants.

Requirements

• Graduate of an accredited NP or PA program with national certification.
• 2–5 years of clinical experience, preferably in primary care or urgent care.
• Valid DEA number.
• Ability to travel weekly between Lost Hills, Delano, and mobile clinic locations.
• Proficiency with Microsoft Office, tablets, smartphones, and EMR systems.
• Strong background in primary care and ability to work independently and as part of a team.

Nice to have

• Experience managing long-term diabetic patients.
• Written and verbal fluency in Spanish.
• Strong interest in population health.
