Care Manager (Behavioral Health)

Location: Remote in California; California residency preferred; must work PST hours

Salary: $27.02–$48.55 per hour

What you will do

• Assess members’ mental health, substance use, social determinant, and broader behavioral health needs by phone, digitally, at home, or at other sites.
• Develop and monitor personalized care plans for members with high-acuity needs.
• Coordinate access to providers, specialists, community resources, and other required services.
• Collaborate with members, families, caregivers, healthcare providers, and community organizations.
• Track treatment changes, side effects, complications, clinical symptoms, and member outcomes.
• Document care management activities and provide education on treatment options, referrals, healthcare benefits, and provider instructions.

Requirements

• California licensure is required.
• Master’s degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing.
• Licensed Master’s Behavioral Health Professional, such as LCSW, LMSW, LMFT, LMHC, or LPC, or an RN with behavioral health experience, according to state contract requirements.
• 2–4 years of related experience.
• Availability to work PST hours.
