Inpatient Nursing Care Coordinator

Location: 100% onsite in Cambridge, Massachusetts; 8:00 AM–6:00 PM shift, 40 hours per week with rotating weekends.

Salary: $44.14–$109.51 per hour.

What you will do

• Serve as a non-clinical point of contact for high-risk adult oncology patients, families, and caregivers.
• Coordinate appointments, referrals, follow-up activities, and services across primary care, specialty care, and community-based organizations.
• Assess social determinants of health and connect patients with resources addressing transportation, housing, food insecurity, financial stress, and language barriers.
• Collaborate with physicians, nurses, social workers, pharmacists, and community health workers on patient-centered care plans.
• Reinforce education about appointments, medications, self-management, and preventive care while advocating for patient preferences.
• Document patient interactions, care plans, outreach, and referrals in electronic health records and use data for panel management and performance improvement.

Requirements

• Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred.
• At least one year of experience in care coordination, population health, case management, community health work, or a related healthcare or human services role.
• Experience supporting people with complex medical and social needs and navigating healthcare and community-based services.
• Current, unrestricted Massachusetts Registered Nurse license required.
• Basic Life Support certification required. Advanced Cardiovascular Life Support, Oncology Certified Nurse, and case management certifications are preferred depending on the role and work area.
• Strong communication, organization, patient advocacy, confidentiality, electronic health record, and multidisciplinary collaboration skills.
