Care Coordinator III (Medicaid)

Location: Fully remote from Illinois; applicants must reside in Cook, DuPage, Grundy, Kane, Kankakee, Kendall, Lake, McHenry, or Will County. Requires 10% travel to a local office for quarterly team meetings. Work schedule: Monday-Friday, 8:00 a.m.-5:00 p.m. Central Time.

Salary: $20.39-$34.71 per hour.

What you will do

• Conduct telephone outreach and home visits with Illinois Medicaid YouthCare members to discuss care plans, services, resources, questions, and ongoing education.
• Coordinate care activities with members, caregivers, healthcare providers, community organizations, and care managers.
• Perform service assessments and screenings for members with complex needs and document identified care needs.
• Respond to member and provider inquiries, triage complex requests, and escalate issues to management when needed.
• Maintain member records and distribute documentation to providers in accordance with regulatory and contractual requirements.
• Identify member needs and make referrals to care managers, community-based organizations, and disease managers.

Requirements

• Must reside in one of the specified Illinois counties.
• High school diploma or GED required.
• Two to four years of related experience required.
• Bachelor’s degree in nursing, social sciences, social work, or a related human-services field required for the Illinois YouthCare plan.
• One year of supervised clinical experience in a human-services field required for the Illinois YouthCare plan.
• Ability to work Monday-Friday during Central Time business hours and travel to a local office for quarterly meetings.
