Principal Engineer (AI)

Location: In-person at the San Francisco headquarters; partial remote work may be considered for exceptional candidates.

Salary: $250,000–$400,000 per year, depending on experience, plus meaningful early-stage equity.

hirify.global is building an AI-native healthcare platform for autonomous medical care, starting with psychiatry, combining doctors, production AI, clinical data, and end-to-end care operations.

What you will do

• Own complex, load-bearing problem spaces across the care platform from initial investigation through implementation, documentation, and long-term operation.
• Scale the engineering delivery system, including merge gates, migration safety, canary releases, and rollback mechanisms.
• Own the event-sourced clinical work spine for orders, tasks, commands, receipts, auditability, and fail-closed checks.
• Design and operate production LLM agents with tool use, evaluations, observability, and controlled expansion of autonomy in clinical-adjacent workflows.
• Own the data and metrics estate, including the warehouse, analytics platform, and pipelines.
• Set architecture direction, make decisions independently, and communicate technical decisions clearly in writing.

Requirements

• Principal-level experience owning architecture across multiple companies or many codebases, such as a staff-plus individual contributor, chief architect, or platform consultant.
• Experience owning multiple systems end to end and solving ambiguous, high-impact problems independently.
• Daily experience shipping at high volume with AI agents, including production setups and unsupervised workflows.
• Strong written communication and a proactive approach to documenting decisions and implementation details.
• Clinical-grade engineering discipline, including auditability, HIPAA, fail-closed defaults, and invariant-based correctness.
• Production experience with TypeScript, Node.js, Postgres, Next.js, AWS, LLM agent orchestration, EHR or e-prescribing integrations, PHI-grade security, and audit trails.

Nice to have

• Experience building bespoke agent harnesses, tools, CLIs, skills, evaluations, and observability systems.
• Experience scaling engineering organizations or platforms toward substantially higher deployment volume.
