Transition Specialist (LVN/LPN)

Location: Remote in California; PST hours required

Salary: $27.02–$48.55 per hour

What you will do

• Coordinate safe transitions for members moving between healthcare services, community care, adulthood, and adult providers.
• Collaborate with care coordination, care management, providers, and healthcare teams to develop and implement transition, service, and care plans.
• Identify members requiring transition services and help ensure existing authorizations are honored during the transition process.
• Educate members, families, caregivers, providers, and staff about services, requirements, limitations, and exclusions.
• Assess member needs and available resources, facilitate appropriate care plans, and support effective treatment coordination.
• Track transition metrics, maintain documentation, identify trends, and support process improvements and training materials.

Requirements

• Current California LVN/LPN state licensure is required.
• Bachelor’s degree and 2–4 years of related experience, or equivalent applicable experience.
• Availability to work PST hours.
• Experience with charting, case management, discharge planning, and telephonic care coordination is preferred.
