Pharmacist - Case and Disease Management (CMDM) - Remote

Location: Remote within the United States, with the listed work-at-home location in Pennsylvania. Travel requirement: 0%–25%.

What you will do

• Review medication-related gaps in care for high-risk members as part of a multidisciplinary product-line team.
• Gather claims data and research clinical websites to evaluate each patient’s situation.
• Develop clinical recommendations to address gaps in care.
• Contact patients, caregivers, physicians, and pharmacists to coordinate interventions.
• Apply clinical judgment, problem-solving, and decision-making skills while maintaining compliance and confidentiality.

Requirements

• Bachelor’s degree in Pharmacy; a Doctorate in Pharmacy is preferred.
• Registered Pharmacist license required.
• 5–7 years of pharmacy experience, or 1–2 years with completion of a clinical residency program.
• Experience in Pharmacist Utilization Management or Pharmacist Medical Therapy Management is applicable.
• Strong oral and written communication, telephone, collaborative problem-solving, and decision-making skills.
• Must work remotely from the United States; the position includes access to protected health and confidential customer information.
