Registered Nurse Care Manager

Location: Riverside, California, United States. Hybrid work with remote clinical activities and required field-based visits and in-person member engagement across the Riverside, Inland Empire, and San Bernardino areas.

Salary: $95,000–$110,000 per year

What you will do

• Provide clinical oversight and guidance to non-clinical Lead Care Managers within the Enhanced Care Management program.
• Review member charts, medical histories, treatment adherence, and care plans in EMR/EHR systems.
• Identify gaps in care and recommend interventions to improve member health outcomes.
• Coordinate care with Lead Care Managers, Behavioral Health Care Managers, Community Health Workers, providers, hospitals, and community organizations.
• Conduct field-based visits, member assessments, in-person engagement, referrals, and care transition activities.
• Participate in case reviews and provide clinical input for health-plan and regulatory reports.

Requirements

• Active Registered Nurse license in California required.
• Minimum 2 years of nursing experience.
• At least 1 year of experience in care management, case management, or a leadership capacity.
• Experience supporting vulnerable populations and individuals with complex medical and social needs.
• Strong clinical assessment, care planning, communication, collaboration, and clinical judgment skills.
• Ability and willingness to perform field-based work and in-person member engagement required.
