Dedicated Care Coordinator Mountain (Pharmaceutical Access)

Location: United States - Colorado - Remote; remote colleagues work from home five days a week. Mountain Time Zone; approximately 10%–20% travel required. U.S. work authorization is required, and U.S. visa sponsorship is not available.

Annual base salary: $162,900–$271,500, plus a 20% bonus target and eligibility for a share-based long-term incentive program.

What you will do

• Own assigned patient cases from HUB enrollment through post-fill follow-up and act as the primary contact for patients, caregivers, and healthcare providers.
• Conduct benefits investigation and verification, explain coverage and out-of-pocket costs, and support prior authorizations and appeals.
• Coordinate baseline assessments, patient appointments, vouchers, Interim Care, copay programs, prescription triage, and fulfillment.
• Manage patient cases, documentation, consents, communications, and prescription workflows in Polaris and related systems.
• Collaborate with HUB partners, vendors, payers, providers, and internal cross-functional colleagues to resolve access barriers.
• Report adverse events and maintain compliant, confidential communication throughout the patient journey.

Requirements

• B.S. or B.A. degree and 8+ years of pharmaceutical or related customer-facing experience.
• At least six months of experience in an FRM, PAC, FAS, or similar access and reimbursement role using the Polaris CRM system.
• Working proficiency in Polaris, including case management, documentation, prescription triage, and interpretation of prior authorization, appeal, and patient enrollment documentation.
• Experience with benefits investigation and verification, prior authorizations, appeals, copay programs, vouchers, and Interim Care or free-goods programs.
• Ability to manage a high volume of concurrent cases, communicate directly with patients and providers, and meet adverse-event reporting obligations.
• Permanent U.S. work authorization is required; visa sponsorship is not available.

Nice to have

• Experience in patient-facing access and reimbursement education or working with a HUB.
• Experience with Kiteworks, DocuSign, copay portal administration, IQVIA/MDX, Sonexus, or similar vendors.
• Experience supporting Interim Care, free-goods, bridge programs, and both pharmacy benefit and major medical coverage pathways.
