Care Manager (Healthcare)

Location: Field-based throughout Denver, Weld County, and Boulder County, Colorado; requires fieldwork and frequent travel to patients’ homes 5 days per week

Salary: $23.00–$26.00 per hour

What you will do

• Schedule and conduct quarterly supervisory visits for home-care compliance.
• Perform home inspections to assess participant safety and environmental compliance.
• Support members and caregivers with authorized care hours, service plans, and ongoing care needs.
• Monitor caregiver daily notes and identify training or support needs.
• Communicate health, safety, and unmet-care concerns to the staff RN and Case Manager.
• Document member and caregiver interactions and field activities in Salesforce and the CRM.

Requirements

• Bachelor’s degree or equivalent work experience.
• Two or more years of experience as a bedside CNA, QMAP, Direct Care Worker, Medical Assistant, or in a similar role.
• At least one year of call center, telephonic outreach, customer service, or related experience.
• Strong verbal and written communication, rapport-building, problem-solving, critical thinking, and time-management skills.
• Ability to work independently, identify concerns, escalate issues, and maintain patient confidentiality under HIPAA guidelines.
• Comfort with computer software navigation and a fast-paced startup environment.
