Care Coordinator (Healthcare)

Location: Remote within Arizona, Florida, Georgia, or Texas; occasional travel may be required for team meetings and company events.

Salary: $17.91–$23.51 per year.

What you will do

• Serve as the primary entry point for members and providers working with the Care Management team.
• Handle inbound calls, route requests through IVR pathways, and pursue one-call resolution.
• Coordinate care for members across complex care management, transitions of care, unassigned populations, hospitals, clinics, home care, vendors, and dialysis settings.
• Arrange non-emergency medical transportation through UberHealth while following program guidelines.
• Manage escalations to Claims, Eligibility, Care Navigation, and other specialized departments.
• Lead special projects as a subject matter expert and collaborate across departments to improve the member experience.

Requirements

• At least 1 year of experience in care coordination and navigation within a healthcare setting.
• Strong computer literacy and proficiency with EMR and telephony systems, including tools such as Medecision and InContact.
• Excellent interpersonal communication and listening skills.
• Residence in Arizona, Florida, Georgia, or Texas is required for this remote role.

Nice to have

• Medical Assistant certification.
• Bilingual communication skills.
• Experience with health insurance.
