Visiting Nurse Association - Licensed Nursing Assistant I - Hospice Care (Hospice)

Location: Manchester, New Hampshire; travel throughout New Hampshire is required for home health care services.

What you will do

• Provide delegated nursing, homemaking, personal care, and other assigned services to patients and families in homes or designated facilities.
• Follow the patient/client Home Health Aide care plan and maintain a healthy, safe environment.
• Perform patient care and treatments, including ordered PT/INR checks in the home, and report results according to policy.
• Reinforce teaching from the primary nurse or therapist with patients and caregivers.
• Complete clinical records, timesheets, and quality control documentation.
• Support peers, students, and other departments in delivering patient care and participate in quality improvement.

Requirements

• Current Nursing Assistant License in the State of New Hampshire is required.
• High school diploma or equivalent preferred.
• Six months to one year of related experience desired.
• Valid driver's license and automobile insurance meeting established EHS minimums are required.
• Availability for 8- to 12-hour days, including rotating weekends, evenings, nights, on-call coverage, and coverage for absences.
