Dedicated Case Coordinator (Patient Access)

Location: United States — Pennsylvania — Remote. Remote colleagues work from home five days a week. Eastern Time Zone alignment is required, with approximately 10–20% travel.

Annual base salary: $162,900–$271,500, plus a 20% bonus target and eligibility for a share-based long-term incentive program.

What you will do

• Own assigned patient cases from HUB enrollment through post-fill follow-up, serving as the primary contact for patients, caregivers, and healthcare providers.
• Conduct benefits investigation and verification, explain coverage and out-of-pocket costs, and support prior authorization and appeals.
• Coordinate baseline assessments, patient appointments, testing, prescriptions, vouchers, Interim Care, and specialty pharmacy fulfillment.
• Administer copay support, verify eligibility, manage copay card access, and follow up after prescription fulfillment.
• Maintain complete and compliant case documentation in Polaris, including patient communications, prescription triage, consents, and partner coordination.
• Report adverse events, protect patient confidentiality, and collaborate with internal teams, HUB partners, payers, providers, and vendors.

Requirements

• Bachelor’s degree and 8+ years of pharmaceutical or related customer-facing experience.
• At least six months of experience as an FRM, PAC, FAS, or in a similar access and reimbursement role using Polaris CRM.
• Working proficiency in Polaris case management, documentation, prescription triage, and interpretation of prior authorization, appeal, and patient enrollment documentation.
• Experience with benefits investigation and verification, payers, prior authorization, appeals, copay programs, vouchers, and Interim Care or free-goods programs.
• Ability to manage a high volume of concurrent cases, communicate directly with patients and providers, and meet adverse event reporting obligations.
• Must be authorized to work in the United States by any employer and hold permanent U.S. work authorization. U.S. visa sponsorship is not available.

Nice to have

• Experience working with a HUB, specialty pharmacy, patient assessment vendors, or third-party logistics providers.
• Experience with Kiteworks, DocuSign, copay portals, Interim Care, free-goods, bridge programs, or both pharmacy and major medical benefit pathways.
