Behavioral Health Outreach Care Specialist Lead (Behavioral Health)

Location: Field-based role serving Vancouver, Clark County, Klickitat County, and Skamania County, Washington; occasional travel within these counties and office attendance for meetings or training. Candidates must live within reasonable commuting distance of the posting locations.

Salary: $30–$45 per hour

What you will do

• Coordinate follow-up care plans, appointments, member programs, and referrals to local, state, and federally funded services.
• Assess compliance with treatment plans by telephone and through on-site member visits.
• Identify barriers to treatment-plan compliance and coordinate resolutions with case management and providers.
• Recommend treatment-plan modifications and additional services for complex cases.
• Coach members on reducing health risks and document case and compliance updates.
• Serve as a team lead and participate in cross-functional projects and process-improvement initiatives.

Requirements

• High school diploma or equivalent and 5 years of related experience, or an equivalent combination of education and experience.
• Strong oral, written, interpersonal, problem-solving, facilitation, and analytical skills.
• Ability to travel to client sites or designated locations within the assigned counties.
• Ability to work in a field-based role with occasional office attendance.

Nice to have

• Behavioral health or mental health experience.
• Certified nursing assistant or certified medical assistant credentials.
• Bachelor’s degree in a related field.
• Bilingual communication skills.
