Home Health RN - Per Diem

Location: North Miami, Florida, United States; patient-facing visits are performed in patients’ homes.

Salary: $77,200–$106,200 per year full-time equivalent; compensation varies based on location, skills, experience, education, and certifications.

What you will do

• Provide skilled nursing care in patients’ homes according to physician orders and individualized plans of care.
• Complete initial and ongoing patient evaluations, determine nursing needs, and develop, initiate, and revise plans of care.
• Perform specialized nursing, preventive and rehabilitative procedures, infection-control practices, and total patient care as needed.
• Prepare clinical notes, progress documentation, care-conference summaries, OASIS assessments, ICD-10 coding, orders, certifications, recertifications, and discharge summaries.
• Coordinate care with physicians, patients, families, physical therapists, speech-language pathologists, occupational therapists, medical social workers, home health aides, and LPNs/LVNs.
• Take assigned on-call shifts during nights, weekends, and holidays and conduct required supervisory visits.

Requirements

• Active Registered Nurse license in Florida required.
• CPR, OSHA, HIV/AIDS, HIPAA, Alzheimer’s, Medical Errors, and Domestic Violence certifications required.
• At least one year of professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors.
• Valid in-state driver’s license, current vehicle registration and insurance, and no more than two driving-record points within the last three years required.
• Personal vehicle liability insurance of at least 100,000/300,000/100,000 required.
• Patient-facing work requires TB screening and Florida Level 2 background screening.
