Field Nurse (Per Diem / Queens, NY)

Location: Field-based position in Queens, New York, requiring frequent travel to patients' homes and travel within the five boroughs.

Compensation: $75–$120 USD per assessment

What you will do

• Conduct initial and reassessment evaluations to determine client care needs and develop individualized care plans.
• Train and supervise non-skilled caregivers providing personal care, companionship, and daily living assistance.
• Monitor client well-being, identify changes in health status, and coordinate communication with healthcare professionals.
• Educate clients and families on care strategies, safety measures, and daily living skills.
• Maintain accurate, confidential documentation and ensure compliance with agency policies and state and federal regulations.
• Coordinate with clients, families, caregivers, and healthcare professionals to improve quality of care.

Requirements

• BSN with a current New York State or compact nursing license in good standing, or an associate degree in nursing with relevant homecare clinical experience.
• At least 1–2 years of nursing experience, preferably in a homecare agency, including experience with geriatric patients or individuals with disabilities.
• Ability to travel within the five boroughs, with a valid driver's license and reliable transportation.
• Knowledge of state and federal home health regulations and strong experience using a Uniform Assessment System.
• Proficiency with electronic health record systems and Microsoft Office Suite.
• Strong interpersonal, written and verbal communication, organizational, and time-management skills.

Nice to have

• Master's degree in nursing.
• Experience in home care, geriatrics, case management, or field-based patient care assessment.
• Three or more years of homecare agency experience.
• Bilingual ability in Spanish, Russian, Chinese, Korean, or Bengali.
