LTC Wellness Manager (Long-Term Care)

Salary: $75,500–$142,700 per year

What you will do

• Own the execution, evolution, and continuous improvement of complex long-term care wellness programs.
• Develop roadmaps, implementation plans, operating models, workflows, decision frameworks, and success measures.
• Use clinical, functional, cognitive, behavioral, environmental, and social insights to shape member journeys, assessments, interventions, and service models.
• Conduct or oversee telephonic engagement and assessments for higher-risk or complex members.
• Establish performance indicators and partner with Analytics and Finance to evaluate outcomes, trends, effectiveness, and business value.
• Manage wellness vendors, lead governance discussions, coordinate operational readiness, and influence Product, Technology, Digital Experience, and Operations teams.

Requirements

• Bachelor’s degree in Occupational Therapy, Nursing, Social Work, Gerontology, Public Health, Rehabilitation, Case Management, Health Administration, or a related clinical or health discipline.
• 7+ years of progressive experience in clinical services, care management, wellness, aging services, rehabilitation, population health, insurance, or long-term care.
• 3+ years leading complex programs, implementations, pilots, products, operational improvements, or cross-functional initiatives.
• Active Occupational Therapist (OTR/L) licensure is strongly preferred; Physical Therapist licensure is highly valued.
• Expertise in functional assessment, aging in place, home safety, caregiver support, mobility, independence, care navigation, and related aging needs.
• Experience influencing stakeholders without direct authority, evaluating performance data and financial considerations, managing ambiguity, and communicating recommendations to senior leaders.

Nice to have

• Master’s degree in a related clinical, public health, business, or health administration discipline.
• Experience managing external vendors, service partners, contracts, implementations, or performance governance.
• Experience with CRM, workflow, case management, analytics, or digital engagement platforms.
