Care Manager RN

Location: Remote in Florida with 10% travel to a local office for quarterly team meetings. Schedule: Monday–Friday, 8:00am–5:00pm Eastern Time.

Salary: $56,200–$101,000 per year

What you will do

• Assess member needs, barriers to care, social determinants of health, and personal priorities.
• Develop and update personalized care plans and service plans for Medicaid and Medicare members.
• Coordinate with members, families, caregivers, providers, specialists, and community resources.
• Monitor member status, changes in condition, care gaps, and progress toward goals.
• Provide education on diseases, healthcare options, referrals, provider instructions, and benefits.
• Document care management activities and support compliance with state, federal, and payer requirements.

Requirements

• Must reside in Florida.
• Active, unrestricted Florida RN licensure is required.
• Degree from an accredited school of nursing or a Bachelor’s degree in Nursing.
• 2–4 years of related experience.
• RN state licensure and/or compact state licensure.
