What you will do

• Lead, mentor, recruit, train, and retain case management professionals.
• Oversee case management services, including assessment, planning, referrals, coordination, monitoring, consultation, and advocacy.
• Drive program development, continuous improvement, quality assurance, and service outcomes.
• Monitor compliance with company, state, and federal requirements.
• Build relationships with counties, community agencies, and other stakeholders.
• Support strategic planning, budgeting, policy development, committees, conferences, and industry initiatives.

Requirements

• Bachelor’s degree in a human service-related field.
• At least two years of experience working with individuals with disabilities or older adults.
• At least three years of supervisory experience in case management services.
• Knowledge of case management principles, procedures, practices, and the local community.
• Strong written and oral communication, analytical, negotiation, conflict-resolution, and adaptability skills.
• Valid driver’s license and proof of automobile insurance.
