Case Manager

Location: Remote Flex - North Carolina; remote work with a few in-office visits each year. The role may be performed in states listed on the posting, with onsite expectations discussed during the interview process.

Salary: $73,698.00–$117,917.00 annually, plus potential annual incentive bonus.

What you will do

• Coordinate care, resources, and services for selected member populations as part of a multidisciplinary team.
• Complete comprehensive assessments and develop member-centric treatment, intervention, and discharge plans.
• Monitor member progress, modify care goals, provide education, and coordinate referrals and community resources.
• Facilitate care across physician offices, hospitals, rehabilitation units, skilled nursing facilities, and home care.
• Evaluate medical necessity, treatment alternatives, benefit options, and appropriate utilization of health care resources.
• Document assessments, care plans, interventions, monitoring, outcomes, and relevant policies or training materials.

Requirements

• Registered Nurse with at least 3 years of clinical and/or case management experience.
• Valid and applicable North Carolina or compact multi-state clinical licensure is required and must be maintained.
• Certified Case Manager (CCM) certification must be obtained within 2 years of employment.
• Ability to perform clinical assessments, care coordination, patient education, and health care resource management.

Nice to have

• Case management experience.
• Maternal health or pediatric nursing experience.
• Electronic medical records experience.
• Strong technical aptitude and ability to learn new systems quickly.
• Certified Case Manager (CCM) certification.
