Director, Community Health Operations (Medtech)

Location: Remote within the United States; candidates must be authorized to work in the United States.

What you will do

• Lead community health worker operations and Community Supports through two operational leaders.
• Own workforce deployment, geographic footprint, and hiring plans across twelve counties and eight health plan contracts.
• Integrate community health workers, RNs, and care coordinators into unified care teams during RN-led care model conversions.
• Own audit accountability, health plan fidelity requirements, DHCS readiness, and operational performance reporting.
• Manage unit economics for Community Supports and the CHW Benefit in partnership with Finance.
• Build career pathways, training frameworks, management structures, and decision rights for a growing frontline workforce.

Requirements

• Eight or more years leading community health, care management, or population health programs, including management through other leaders.
• Experience operating distributed frontline teams across multiple counties or markets.
• Medicaid experience; CalAIM is strongly preferred, with Enhanced Care Management or Community Supports experience highly valuable.
• Experience integrating community-based teams into clinical care delivery.
• Ability to use operational data for decisions and defend those decisions to health plans.
• Experience with audit and regulatory readiness in a Medicaid setting, plus designing workflows and management structures for a growing frontline workforce.

Nice to have

• Experience in a tech-enabled delivery organization during operational change.
• Experience designing career pathways and training frameworks for frontline staff.
• Experience with housing navigation or other social services delivered under a Medicaid benefit.
• Bilingual Spanish or experience leading teams serving predominantly Spanish-speaking members.
• Advanced degree in public health, social work, healthcare administration, or a related field.
