Field Adv Practice Provider

Location: Field-based role in Atlanta, Fayetteville, or Stockbridge, Georgia, United States; candidates must live within a reasonable commuting distance of the posting location. Travel to patient homes, client sites, or designated locations is required, with occasional office attendance for meetings or training.

What you will do

• Provide clinical and non-clinical palliative care services in patients' homes.
• Develop and implement care and treatment plans for adults with chronic and complex medical and mental health conditions.
• Conduct patient histories, physical examinations, and diagnostic assessments as needed.
• Address patients' and families' physical and psychosocial needs in collaboration with an interdisciplinary team.
• Educate patients and families about medications, illness progression, nutrition, adherence, and crisis prevention.
• Coordinate care with physicians, medical providers, patients, and families while maintaining compliant medical records and prescribing medication within state authority.

Requirements

• Master of Science in Nursing.
• Current unrestricted Nurse Practitioner or Physician Assistant license in the applicable state.
• DEA registration or eligibility and CPR certification.
• Ability to work in a field-based role and travel to patient homes and designated locations.
• Completion of a tuberculosis test is required for the Carelon Health business unit.

Nice to have

• Two years of experience in home-based services, complex care, hospice, or palliative care.
• Bedside hospital, oncology, emergency room, or intensive care unit experience.
• Experience with electronic medical records.
• Bilingual English/Spanish communication skills.
