Bilingual (Spanish/English) Transitions of Care Advanced Practice Provider

Location: Remote USA; Monday–Friday, 8:30 AM–5:00 PM, with one weekend shift per month. CST, PST, or MST preferred.

Salary: $121,000–$171,720 annually.

What you will do

• Conduct comprehensive post-discharge assessments covering medical conditions, medications, functional status, and psychosocial needs.
• Complete medication reconciliation, place prescriptions and clinical orders, arrange referrals and services, and provide disease-specific education and contingency plans.
• Provide virtual urgent care for acute symptoms, including diagnosis, treatment, and appropriate escalation.
• Participate in interdisciplinary rounds and collaborate with RNs, LPNs, nurse case managers, guides, and clinical coordinators.
• Support clinical education, quality improvement projects, pilots, and the development of new virtual care initiatives.
• Use the electronic health information system and provide feedback to improve its interface.

Requirements

• Spanish and English bilingual proficiency required.
• At least three years of experience in a hospital, post-hospital, or primary care setting.
• Active RN and APRN licenses in good standing, or an active Physician Assistant license in good standing.
• Ability to obtain licensure in required Devoted states within 90 days of hire.
• Active BLS certification at hire and throughout employment.
• Ability to work full time in a fast-paced, interdisciplinary virtual care environment.

Nice to have

• Experience in internal medicine, urgent care, emergency medicine, hospital medicine, post-acute medicine, or geriatrics.
• Telehealth video visit experience.
• Experience managing post-transition patients and acute or chronic disease exacerbations, including CHF, diabetes, COPD, and hypertension.
