Family Nurse Practitioner or Physician Assistant (Primary Care)

Location: New Haven, Indiana, United States; onsite primary care position

Salary: $115,000–$150,000 per year; $5,000 sign-on bonus

What you will do

• Provide relationship-based family and primary care at the top of the license.
• Deliver preventive care, wellness services, chronic disease management, health education, and urgent visits.
• Build lasting relationships with patients and clients through personalized care.
• Work autonomously with clinical resources including UpToDate and RubiconMD.
• Focus on health outcomes rather than patient volume or fee-for-service billing.

Requirements

• Active professional license and current ANCC or AANP board certification for Nurse Practitioners, or NCCPA board certification for Physician Assistants.
• CPR/BLS certification required by the start date.
• Independent practice authority is strongly preferred where applicable under state regulations.
• Federal DEA number preferred and potentially required for full prescribing ability.
• Independent family practice experience in wellness care, chronic care management, and urgent visits preferred.
• Work is based onsite in New Haven, Indiana.
