Medical Coder (Healthcare)

What you will do

• Review and abstract professional medical records, provider notes, encounters, and supporting documentation.
• Assign ICD-10-CM, CPT, HCPCS, and applicable modifiers according to national and payer-specific guidelines.
• Validate code support and query providers when clarification is required.
• Maintain accuracy, productivity, and compliance metrics.
• Participate in internal and external coding audits and provide process improvement feedback.
• Track updates to CPT, ICD-10, HCPCS, and CMS risk adjustment guidelines while protecting patient confidentiality.

Requirements

• Active AAPC certification such as CPC or COC, or AHIMA certification such as CCS-P or CCS.
• At least 3 years of professional-fee coding experience.
• Strong knowledge of HCC and risk adjustment coding principles, medical terminology, anatomy, physiology, pathophysiology, disease progression, and pharmacology.
• Deep familiarity with CPT, ICD-10-CM, HCPCS, and modifier assignment.
• Ability to work independently and maintain productivity in a remote setting.
• Experience with EHR systems, encoder or coding software, Google tools, and a reliable internet connection with a dedicated secure workspace.
