Certified Diabetes Patient Educator (Virtual Remote) -West

Location: Phoenix, AZ; remote work from a dedicated workspace in the primary home residence. Candidates must be authorized to work in the United States without visa sponsorship.

Salary: $102,000–$113,000 annually.

What you will do

• Educate patients about specific diabetes treatments, prescribed medications, and related resources.
• Conduct one-on-one communications with enrolled patients by telephone, video, email, and SMS.
• Support patient persistence, treatment compliance, and effective diabetes management.
• Follow all program guidelines while maintaining accurate documentation.
• Build relationships with patients and cross-functional partners.
• Manage priorities, deadlines, and multiple projects independently in a fast-paced environment.

Requirements

• Bachelor’s degree and/or BSN.
• 3+ years of experience as a Certified Diabetes Educator (CDE) or Certified Diabetes Care and Education Specialist (CDCES).
• Complete understanding and application of diabetes care principles, practices, and standards directly with patients.
• Experience with direct patient interaction and support for treatment persistence and compliance.
• Strong presentation, interpersonal, written, verbal, virtual communication, listening, and problem-solving skills.
• Proficiency in Microsoft Office, including Teams, Word, Excel, PowerPoint, and SharePoint.

Nice to have

• Registered Nurse, Registered Dietitian, Physician Assistant, Nurse Practitioner, Pharmacist, or another relevant healthcare certification.
• Contact center experience using a telephony platform.
• Motivational Interviewing experience.
• Bilingual fluency in Spanish and English.
