CHC Practice Transformation Specialist (Healthcare)

Location: Remote, but candidates must be based in South Carolina; travel to South Carolina practices up to 25%.

Salary: $65,000–$75,000 per year base, plus bonuses and equity.

What you will do

• Implement hirify.global products and execute ACO initiatives under market leadership direction.
• Support wellness visits, transitions of care, active patient management, and practice transformation plans.
• Train office staff, physicians, and providers on clinical processes and technologies.
• Assess ongoing training needs and build relationships with practices, providers, and healthcare executives.
• Listen to practice concerns and communicate improvement opportunities to managers and leads.

Requirements

• Must be a resident of South Carolina.
• Graduate of an accredited college or university, or 4–6 years of relevant medical practice management experience in lieu of a degree.
• At least 3 years of experience using electronic health records for clinical or practice management processes.
• Strong oral and written communication, presentation, organizational, project management, and team-building skills.
• Ability to travel to practices in South Carolina up to 25% of the time.
• Comfort with computers and willingness to learn additional software applications.

• Familiarity with process mapping and workflow analysis tools.
