Travel Family Nurse Practitioner or Physician Assistant (Primary Care)

Location: IN-South Bend; travel required across South Bend, Mishawaka, Elkhart, Angola, Butler, and LaGrange, Indiana

Salary: $55.00–$72.00/hour

What you will do

• Provide relationship-based family primary care at the top of the license.
• Deliver preventive care, wellness services, chronic disease management, urgent visits, and health education.
• Travel between health center locations to cover absent providers and open positions on short notice.
• Collaborate with different teams and adapt to varied clinical settings.
• Maintain continuity of care, consistent procedures, and effective task transitions.

Requirements

• Active state license and current ANCC or AANP board certification for Nurse Practitioners, or NCCPA board certification for Physician Associates.
• CPR/BLS certification required by the start date.
• Independent practice provider status preferred where permitted by state regulations.
• Federal DEA number preferred and potentially required for full prescribing authority.
• Independent family practice experience in wellness care, chronic care management, and urgent visits preferred.
• Ability to maintain a flexible schedule and travel between multiple locations is required.
