Practice Transformation Specialist (Maryland/Delaware)

Location: Hybrid; candidates must be based in Maryland, Delaware, or the DMV area. Travel up to 25% across the Maryland/Delaware market is required for local practice visits.

Salary: $75,000–$80,000 per year base, plus bonuses and equity.

What you will do

• Implement hirify.global products and ACO initiatives under the direction of market leadership.
• Support wellness visits, transitions of care, active management, and practice transformation plans.
• Train office staff, physicians, and providers on value-based care processes and technologies.
• Assess practice training needs and build lasting relationships with providers and healthcare executives.
• Listen to practice concerns and communicate improvement opportunities to managers and leads.
• Support quality and financial performance across relevant metrics.

Requirements

• Residency in Maryland, Delaware, or the DMV area is required.
• Graduate of an accredited college or university, or 4–6 years of significant relevant medical practice management experience in lieu of education.
• At least 3 years of experience with electronic health records for clinical or practice management processes.
• Strong oral and written communication, organizational, project management, and team-building skills.
• Willingness to travel up to 25% for local practice visits.
• Comfort with computers and learning additional software applications.

Nice to have

• Experience with process mapping and workflow analysis tools.
• Significant administrative experience in a clinic setting.
• Public presentation experience.
