Practice Transformation Specialist (Healthcare)

Location: Hybrid in Baltimore, Bethesda, Newark, or Annapolis; candidates must be based in Maryland, Delaware, or the DMV area. Travel up to 25% across the Maryland/Delaware market is required for local practice visits.

Salary: $75,000–$80,000 per year base, plus bonuses and equity.

What you will do

• Implement products and execute ACO initiatives under the direction of market leadership.
• Support wellness visits, transitions of care, and active management while helping practices meet mutually developed transformation-plan goals.
• Train office staff, physicians, and providers on healthcare processes and technologies in coordination with the value-based care curriculum.
• Assess ongoing training needs and build lasting relationships with physicians, providers, and healthcare system executives.
• Support improvements in financial and quality performance across relevant metrics.
• Listen to practice concerns and communicate improvement opportunities to managers and leads.

Requirements

• Must be a resident of Maryland, Delaware, or the DMV area.
• Graduate of an accredited college or university, or 4–6 years of significant and relevant medical practice management experience in lieu of the educational requirement, particularly significant administrative experience in a clinic setting.
• At least 3 years of experience with Electronic Health Records for clinical or practice management processes.

Nice to have

• Excellent oral and written communication, public presentation, organizational, project management, and team-building skills.
• Familiarity with process mapping and workflow analysis tools.
• Excellent computer skills and willingness to learn additional software applications.
