TL;DR
Enrollment RN (PACE healthcare): Conducting clinical eligibility, nursing facility level-of-care, and home safety assessments for prospective older adult participants, with an accent on participant education, accurate documentation, and interdisciplinary enrollment decisions. Location: Compton, California, United States; requires travel throughout the service area and visits to participants' homes, care environments, and community locations.

Salary: $43–$53 per hour, plus a target annual bonus and potential additional incentive compensation.

What you will do

  • Serve as the clinical lead throughout the participant enrollment process.
  • Conduct comprehensive nursing, clinical eligibility, and home safety assessments in participants' homes and other care environments.
  • Complete Nursing Facility Level of Care assessments and documentation in accordance with DHCS requirements.
  • Present clinical findings during enrollment eligibility meetings and collaborate with the Interdisciplinary Team on enrollment recommendations.
  • Educate prospective participants and caregivers, answer clinical questions, and build trusted relationships.
  • Maintain accurate EHR records, support outreach activities, and identify opportunities to improve enrollment workflows and participant experience.

Requirements

  • Associate's degree in Nursing from an accredited program or equivalent nursing education.
  • Active, unrestricted Registered Nurse license in the applicable state.
  • At least one year of nursing experience with older adults, frail elderly populations, or people with complex medical needs.
  • Ability to perform clinical and home safety assessments, apply sound clinical judgment, and manage multiple priorities.
  • Experience with electronic health records and clinical documentation systems, plus strong communication and interdisciplinary collaboration skills.
  • Valid driver's license, reliable personal transportation, automobile insurance, and ability to travel throughout the service area are required. Active CPR/BLS certification is required or must be obtained within 30 days of hire.

Nice to have

  • Bachelor's degree in Nursing.
  • Experience in PACE, Medicare, Medicaid, managed care, or value-based healthcare.
  • Knowledge of PACE, CMS, DHCS, and related federal and state eligibility and enrollment requirements.
  • Experience conducting NFLOC, home safety, or other clinical eligibility assessments.
  • Spanish, Mandarin, or Cantonese bilingual ability.
