Nurse Navigator RN (Healthcare)

Location: Tufts Medical Center; the role involves direct patient care and transitions across inpatient, home health, skilled nursing, and rehabilitation settings.

Salary: $84,300.22–$107,481.20 per year

What you will do

• Coordinate transitions of care with Case Management across inpatient, home health, skilled nursing, and rehabilitation settings.
• Educate patients and families about clinical conditions, treatment, postoperative recovery, and their role in care.
• Advocate for patients, coordinate services and resources, and support appropriate levels of care.
• Use tele-monitoring to support and monitor high-risk patients and help prevent avoidable emergency department visits and rehospitalizations.
• Collaborate with multidisciplinary providers, hospital teams, liaisons, case managers, and community services.
• Maintain intervention records, prepare management reports, and ensure compliance with regulatory and HIPAA/HITECH requirements.

Requirements

• Associate's degree in Nursing.
• Registered Nurse (RN) license.
• Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification.
• At least three years of clinical experience caring for patients with complex medical diagnoses.
• Valid state-issued driver's license and reliable transportation.
• Ability to communicate effectively with patients, families, physicians, and healthcare professionals and manage complex care plans.

Nice to have

• Bachelor's degree in Nursing.
• Advanced Practice Registered Nurse (APRN) qualification.
• Eligibility for state and federal controlled substances registration.
• Five years of experience in home health, post-acute, or acute care settings.
• Bilingual communication skills.
