Weekend Intake Registered Nurse, RN

Location: Lowell - Rogers Street; hybrid/remote. Weekend schedule: Saturday and Sunday, 8:00 a.m.–4:30 p.m. Candidates must hold an active RN license in the state where services are provided and may not work in states where they are not licensed.

Pay range: $42.00–$47.41 per hour.

What you will do

• Review home health and hospice referrals and collect the patient, medical, financial, and referral information required for intake.
• Assess home care eligibility, homebound status, medical necessity, patient safety, and appropriateness of requested services.
• Document referrals in Request for Service forms, Forcura, the EMR, e-tracking systems, and referral orders.
• Prioritize referrals based on clinical urgency, patient safety, and insurance precertification requirements.
• Obtain prior authorizations, verify atypical insurance coverage, and coordinate with billing and clinical teams.
• Resolve referral issues, recommend community resources when appropriate, and communicate acceptance decisions and workflow concerns.

Requirements

• Active RN license required in the state where services are provided.
• Additional state licensure must be obtained within 60 days when required.
• At least two years of visiting nurse experience.
• Clinical judgment, critical thinking, problem-solving, organization, reliability, and the ability to work effectively in busy situations.
• Ability to remain stationary for approximately 90% of the workday, use standard office equipment, and occasionally lift or move 10–25 pounds.
