Licensed Vocational Nurse (LVN)

Location: South LA PACE Center, 1005 E Rosecrans Ave., Compton, CA 90221; travel across the service area and participant homes is required.

Salary: $30.00–$34.00 per hour, plus a target annual bonus of 5% and potential additional incentive compensation.

What you will do

• Provide direct nursing care within LVN scope, including medication administration, injections, vital-sign monitoring, wound care, and ordered treatments.
• Conduct triage, nursing observations, and assessments; identify changes in condition and escalate concerns to registered nurses or providers.
• Implement individualized care plans and support chronic disease management, intake activities, and ongoing participant care.
• Coordinate care and transitions across the PACE center, clinic, homes, external providers, and community settings.
• Educate participants and caregivers on medications, nutrition, hygiene, chronic conditions, safety, and self-management.
• Maintain accurate EMR documentation and contribute to quality, infection prevention, safety, and continuous improvement initiatives.

Requirements

• Completion of an accredited vocational or practical nursing program and an active, unrestricted LVN or LPN license applicable to the state of practice.
• At least one year of nursing experience; experience with older adults or complex medical and functional needs is preferred.
• Ability to provide direct nursing care, apply clinical protocols, recognize changes in condition, and escalate concerns appropriately.
• Strong communication, collaboration, organization, and clinical documentation skills, including experience with EMRs.
• Active CPR/BLS certification or ability to obtain it within 30 days of hire.
• Valid driver's license, reliable personal transportation, automobile insurance, and ability to travel throughout the service area are required.
• Participant-facing health requirements include communicable disease screening, TB clearance, immunizations, infection-control procedures, and applicable vaccination or declination documentation.
• Employment eligibility in the United States is verified through E-Verify.

Nice to have

• Experience in PACE, geriatrics, home health, adult day health, integrated care, or value-based care.
• Geriatric certification, specialized older-adult care training, or bilingual skills.
