Community Health Worker (Healthcare)

Location: Hybrid role supporting the Southern Region, including Cape Cod, with limited travel in the region.

Pay range: $21.53–$26.91 per hour

What you will do

• Help patients navigate healthcare systems, community services, transportation assistance, and other resources.
• Conduct structured assessments during home visits, set goals, and motivate patients to improve their health.
• Provide patient education, social support, informal counseling, and culturally competent advocacy.
• Assist with applications for medical assistance, SNAP, and other financial or community-based resources.
• Coordinate with medical providers, care coordinators, social workers, and interdisciplinary care teams.
• Manage an assigned patient caseload, document contacts in the EMR within 48 hours, and report high-risk concerns.

Requirements

• High school diploma or equivalent.
• At least one year of experience in a healthcare-related setting such as a hospital, home health provider, or community-based agency.
• Valid state-issued driver’s license and reliable transportation; transporting patients is not permitted.
• Ability to work with people from varied ages, backgrounds, cultures, and life experiences.
• Strong interpersonal, communication, customer service, organizational, and multitasking skills.
• Knowledge of medical terminology and proficiency with Microsoft Office and business software.

Nice to have

• Associate’s degree in public health, healthcare, or another health science.
• Three years of experience in a healthcare-related field.
• Bilingual ability and experience in multicultural or community-based settings.
• Familiarity with and connection to the target community.
