Location: Manchester, New Hampshire, with travel throughout New Hampshire

What you will do

• Provide delegated nursing, homemaking, and personal care services in patients’ homes or designated facilities.
• Follow patient care plans established by nurses and therapists while maintaining a safe, healthy environment.
• Monitor and promptly report changes in patient condition, needs, and responses to case and clinical managers.
• Perform ordered PT/INR checks in the home and report results according to policy.
• Document care in clinical records, timesheets, and quality-control logs.
• Reinforce personal-care instruction for patients and caregivers and promote dignity and emotional support.

Requirements

• Current Nursing Assistant License in the State of New Hampshire required.
• High school diploma or equivalent preferred.
• Six months to one year of related experience desired.
• Valid driver’s license and automobile insurance required for travel throughout New Hampshire.
• Availability for shifts of 8–12 hours, including rotating weekends, evenings, nights, and on-call coverage.
