Manager, Care Coaching

Location: Remote from home, with residence required within 1 hour of Black River Falls or Blair, Wisconsin. Occasional travel to hirify.global offices, in-person unit meetings, and quality visits may be required. Working hours are Monday–Friday, 8:00 am–4:30 pm CST.

Salary: $78,400–$107,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Mentor and support Field Nurses, Care Coaches, and IDT staff delivering Family Care benefits in compliance with the DHS contract.
• Identify learning needs, set expectations, and coordinate orientation, mentorship, and ongoing staff development.
• Assess staff competencies and performance standards, review reports and documentation, and evaluate care-management processes.
• Assess utilization and the quality of care management for members served.
• Collaborate across departments and educate community partners to support effective provider services.
• Participate in hiring, selection, orientation, and training; facilitate unit meetings and attend quality visits.

Requirements

• Bachelor’s degree in human services or a related field, or an associate degree in nursing.
• At least 1 year of leadership experience in human services.
• At least 3 years of experience working in the Family Care program.
• Residence within 1 hour of Black River Falls or Blair, Wisconsin is required.
• Ability to work remotely from a dedicated space that protects member PHI and HIPAA information.
• Self-provided internet service with at least 25 Mbps download and 10 Mbps upload speeds.

Nice to have

• Five years of experience working with individuals with intellectual or developmental disabilities, physical disabilities, or frail elders.
