Care Manager (CNA: part-time)

Location: Southbury, Connecticut, USA; onsite in Assisted Living and/or Memory Care

Salary: USD 19.00–22.00 per hour

What you will do

• Provide bathing, dressing, toileting, feeding, grooming, and other daily care according to resident care plans.
• Assist with medication self-administration through queuing, prompting, and reminders under RN or LPN supervision.
• Escort residents to meals and activities and support socialization.
• Maintain resident safety, comfort, dignity, confidentiality, and cleanliness.
• Document care and services in the electronic health record during each shift.
• Monitor and immediately report changes in resident condition; obtain weights and vital signs when requested.

Requirements

• High school diploma or GED.
• At least one year of resident care experience.
• Certified Nursing Assistant or Certified Home Health Aide certification.
• Knowledge and experience working with elderly residents.
• Ability to lift or carry up to 50 pounds and support residents using proper body mechanics.
• Availability for scheduled part-time first- or third-shift work, at least 22.5 hours per week and fewer than 30 hours per week.
