Family Nurse Practitioner OR Physician Assistant (NP/PA)

Location: Greensboro Hub, North Carolina; daily onsite presence Monday–Friday

Salary: $120,000–$140,000 per year

What you will do

• Conduct comprehensive clinical intake visits and provide longitudinal primary or wrap-around care for pediatric and adult members.
• Manage chronic conditions including hypertension, diabetes, and chronic kidney disease, and provide preventive and wellness care.
• Evaluate acute concerns and manage behavioral health conditions including depression, anxiety, alcohol use disorder, and opioid use disorder.
• Perform member outreach, timely follow-up, specialist referrals, and support for individual care-plan goals.
• Capture diagnoses and close quality gaps through accurate HCC, ICD-10, and CPT documentation.
• Collaborate with interdisciplinary care teams in case conferences and contribute to clinical programs, initiatives, processes, and policies.

Requirements

• At least 3 years of patient-facing experience.
• Completion of an accredited Nurse Practitioner or Physician Assistant program with national certification.
• Unrestricted license to practice in the state of employment with hirify.global.
• Board certification, or board eligibility within one year of employment.
• Basic Life Support certification for Healthcare Providers.
• DEA licensure or willingness to acquire it; medical clearance is required for member-facing roles.
