Backend Engineer (AI)

Location: On-site in the San Francisco office, five days per week

Salary: $200K–$285K plus equity

What you will do

• Design and build high-performance, reliable backend systems for large-scale clinical data.
• Build and operate distributed systems and data pipelines for high-volume asynchronous workloads using Kafka and Temporal.
• Develop and maintain integrations with electronic health records and other external healthcare systems.
• Implement complex product workflows for areas including prior authorizations and patient interactions.
• Integrate and operationalize in-house and external clinical AI models in production.
• Contribute across APIs and frontend layers to deliver complete user-facing features.

Requirements

• Strong experience building and shipping backend systems with TypeScript.
• Experience with distributed systems and data infrastructure such as PostgreSQL, Redis, Kafka, and Temporal, or similar technologies.
• Ability to design production systems and manage complexity through durable abstractions.
• End-to-end ownership of problems, from understanding requirements through production delivery.
• Ability to work in person at the San Francisco office five days per week.

Nice to have

• Experience with Kubernetes and infrastructure tooling such as Helm.
